A TikTok video has surfaced on Twitter featuring a trans-identified female explaining how the silicone penis pump implanted into the individual’s fake penis works.

Throughout the video, the biological female refers to the appendage fashioned from forearm skin and flesh as a "ween."

"This is one of my testes," explains the TikToker, holding a plastic bulb up to the camera, showing a forearm bearing the scars of phalloplasty surgery. "This is the actual bulb that is now in my ween."

Viewers learn that this particular model of penis pump is a Coloplast Titan, which according to the company’s website is an "inflatable penile prosthesis [that] is a self-contained, fluid-filled system made from Bioflex and silicone…designed to emulate the look and performance of a natural erection."

The TikToker takes viewers through how the inflatable penis implant functions.

"This is what is in my ‘sack’ sitting next to my testicular implant, and this is what is going to inflate and deflate my 'ween,'" said the biological female who avoided naming male anatomy.

"I will simply use the base of my fingers; I will find it in the sack; I will squeeze until it inflates me ‘ween,’ and then when I’m ready to release it, I will find this little lift right here inside my sack and I will squeeze it and that will allow the fluid to go back into the reservoir that this now hiding attached to my pelvic bone behind my tummy muscles. Holy f**k."

This model of penis pump is actually designed for men who suffer from erectile dysfunction, but it is also used by gender surgeons for phalloplasty procedures. Coloplast is currently working on designing an inflatable penis implant specifically for use in phalloplasty.

Phalloplasty is a very complicated, risky surgery that involves stripping the skin and flesh from a patient’s forearm or thigh and using the tissue to create something that resembles a "neo-penis." Most trans-identified females do not opt to undergo such a radical surgery, which has an extraordinarily high rate of severe post-op complications. Of those that do, not all choose to have a pump implanted.

Some surgeons require that a vaginectomy, the removal of the vagina, be performed before phalloplasty surgery. This is especially recommended if the female patient wishes to urinate from the tip of the constructed appendage.
