A TikTok celebrity gender surgeon who advertises her services to vulnerable young people on social media has made a video promoting eunuch as a valid gender identity that may benefit from surgical intervention.

Dr. Sidhbh Gallagher, the Miami-based gender surgeon who jokingly refers to medically unnecessary bilateral mastectomies as “yeet the teets,” posted a video on her TikTok account in which she explains that “a eunuch is somebody who is assigned male at birth but may not be comfortable with the masculine features.”

Gallagher then goes on to say that these individuals may also benefit from “gender affirmation care, which could mean orchiectomy in some patients.” An orchiectomy is the surgical removal of the testes.

Gallagher classifies eunuchs as “gender diverse individuals,” and explains to her young viewers that there is an entire chapter devoted to people who identify as eunuch in the most recent standards of care released by the World Professional Association for Transgender Health (WPATH).

WPATH’s decision to include eunuch in its Standards of Care 8 (SOC8) was met with intense criticism. The chapter defines eunuch as “the oldest recognised gender outside the binary,” and even goes so far as to claim that children can identify as eunuch.

“Like other gender diverse individuals, eunuch individuals may be aware of their identity in childhood or adolescence,” reads the SOC8 chapter. “Due to the lack of research into the treatment of children who may identify as eunuchs, we refrain from making specific suggestions.”

“While there is a 4000-year history of eunuchs in society, the greatest wealth of information about contemporary eunuch-identified people is found within the large online peer-support community that congregates on sites such as the Eunuch Archive (www.eunuch.org), which was established in 1998,” the chapter continues, providing a hyperlink to the Eunuch Archive which contains child-castration pornography.

“Eunuch Archives hosts over 3,000 pieces of fictional child pornography that detail the rape, torture, and killing of children. In some narratives, children with stunted puberty are raped by doctors. In others, children are castrated by force as part of a sadistic sexual torture ritual,” reported Reduxx last year.

The group Beyond WPATH formed shortly after the release of SOC8.

“With the recent release of its Standards of Care, 8th Edition, the World Professional Association for Transgender Health (WPATH) has undermined its standing as the preeminent authority on health care for gender-questioning youth,” reads the group’s declaration, citing the eunuch chapter as one of numerous reasons the international organization can no longer be respected.

When Sweden broke ranks with WPATH last year, pediatrician Mat Reimer stated that the decision to include eunuch as an innate gender identity even children can possess was a sign that WPATH is not a scientific organization but instead an activist group.

“Had this been published anywhere other than in a supposedly scientific article, it would have been thought that it was satire,” said Reimer. “The fact that the National Board of Health and Welfare's updated guidelines should no longer be based on such an obviously activist organization as WPATH is of course welcome.”

In her video, Gallagher explained to her young followers the health concerns that come with being surgically castrated. As a result of taking away the ability to produce sex hormones, patients can have problems with osteoporosis and cardiovascular disease.

“So usually the patient will supplement on either low-dose testosterone or low-dose oestrogen, and it requires specialised care but sometimes this is an important part of gender affirmation surgery for this group of patients,” said the celebrity gender surgeon.

Last year, Gallagher was reported to Biden’s Federal Trade Commission (FTC) for alleged false promotion of services to minors.

The complaint, filed by five groups led by Genspect, alleges the gender surgeon “engaged in unfair, false, and deceptive practices in the aggressive advertising and marketing to minors of their plastic surgery services, namely mastectomies of healthy female breasts, as proven safe, effective, and medically necessary.”

It goes on to allege that Gallagher’s videos are “part of a brand marketing scheme designed to create an online media persona for Gallagher as an entertaining celebrity surgeon in order to appeal to hundreds of thousands of underage social media users, advertise Gallagher’s ‘gender affirming’ plastic surgery services, and sell them to a vulnerable and impressionable population of children and youth experiencing distress with their gender identity and developing bodies.”

The Eunuch session at the 2022 WPATH conference offered a glimpse of the type of person Gallagher has chosen to advertise her services to.

“One type is men with extreme gender dysphoria; they desire to be not male, to be emasculated, but don't want to be female either. They have a male-to-eunuch gender dysphoria now part of the SOC8,” explained chapter author Dr. Tom Johnson.

“There are others who have a feeling that their genitals are not a part of their—proper part of their body—body integrity dysphoria.”

Johnson’s co-presenter, Dr. Michael Irwig, then told the story of the first eunuch patient he ever treated: a 19-year-old who was living in his parents’ basement, playing video games. He wasn't working; he was spending all his time online, and “may have been on the autism Asperger's spectrum.” Irwig observed the young man “wanted to be pre-pubertal; he wanted a low testosterone level,” and so despite not identifying as eunuch, Irwig deduced that he fell into that population.
